First Visit

Thank you for choosing Vista Medical Group for all your family’s medical care needs. To assist us with your care, please arrive at least 15 minutes early to complete registration forms. You can also download the new patient forms from our site and bring them completed to your appointment. Please bring your insurance card and a photo ID. If you are bringing your child, please bring his/her immunization records as well.
